About the role

We are looking to appoint an exceptional candidate to the role of Mental Health Practitioner in the Leaving Care Team (16+).

An exciting opportunity has arisen within the Leaving Care team for a highly specialist and experienced MH practitioner with a substantial knowledge in the field of child and adolescent mental health, particularly in adolescent and young adult mental health. 


The role will involve working collaboratively with young people’s advisors in the Leaving Care team, CLA health and adults’ services to support young people’s physical, emotional and mental health needs in preparation for transition into adulthood and independence. The main purpose is to initially consult, assess, provide intervention and to signpost or refer onto appropriate services as well as provide consultations to other allied professionals. 


The role will additionally provide professional supervision and guidance for a Wellbeing Support Worker.


The successful candidate will be pivotal in our service development to improve the outcome for care leavers by effectively addressing mental health needs.
