We are recruiting to the Head of Service for Complex Care in the operational team that services our older frail residents and younger residents with disabilities from aged 18 upwards.

 

The Head of Service for Complex Care is a high-profile senior leadership role with responsibility for delivering key statutory duties under the Care Act 2014, including prevention, Safeguarding, Direct Payments, supporting Carers, as well as driving forward our vision for Independent Living. The post holder will need to provide leadership for the transformational changes, embedded in our Adult Social Care Improvement Plan. 

 

We have a hybrid working model and staff and will be supported to do this with mobile IT equipment and hot desk facilities in all offices across the borough.  Professional, practice-based supervision will be provided.

About you:

The post holder will require excellent cross organisational relationship building skills to lead on the transformation of our community services alongside NHS and community partners in the design of locally agreed integrated models of care delivery that support admission avoidance and strengthen residents’ independence at home. They will work on the principles of ‘no wrong front door’ and will build on a neighbourhood model to meet the longer-term needs of our diverse local population. 

 

The post holder will require enhanced leadership skills to continue to develop and improve the team’s culture of performance against key statutory duties. Our aim is to deliver excellent services working in the context of financially challenging times, therefore the post holder will need to work creatively with all stakeholders to continue to transform our service delivery, against our medium and long term financial savings programme. 

 

This post requires registration with Social Care England and an Enhanced DBS check.
