About the role
Do you have experience of working with children with disabilities and complex needs and their families? Do you have experience of case holding and undertaking assessments? Are you passionate about ensuring all children can achieve their full potential?
We are currently recruiting a Family Navigator (18hours per week) to work in our new Stephen Wiltshire Centre, specialist Family Hub. You will support children with disabilities and their families to navigate services and access local support.
You will deliver under 5’s play and stay sessions, specialist session for other age groups and parents support meetings. You will assist in the delivery of parenting programs.
The main work strands include:
- Providing a front line duty function for the Stephen Wiltshire Centre Family hub, monitoring the inbox, answering calls and welcoming and supporting visitors to the centre.
- You will provide navigation, co-ordination, information, and support to parents whose children have additional needs and disabilities, aged 0-18.
- Deliver and co-ordinate a varied offer of groups and activities as part of the specialist hub offer
- Undertaking assessments and action plans to identify and deliver appropriate support
- Working closely and making referrals to partners including Education, Health and Social Care, voluntary sector partners to best support families
- Manage a small caseload of children with longer-term needs for support
- Support children and families through key transitions, including into education.
