The Adult Social Care team in Westminster City Council is a world of extraordinary stories. We love it when our work touches lives.

 

Take Stepping to the Beat, our 3k walk, jog or run for people with learning or physical disabilities or Autism, their families and carers. It’s a life-affirming, music-based event Coreen and Claudia are massively proud of – and they were keen for people to spread the word. So, imagine how thrilled they were when one of our residents, Giro, did just that. Advocating for people with disabilities at a planning meeting for transport in London, he was proudly wearing his event medal. And he decided to use it to encourage others. As he passed it around the room, Giro inspired lots more people with disabilities to get involved. Infectious enthusiasm goes a long way here. 

 

The Role:

As a Placement Monitoring Officer you’ll ensure good standards of residential care and supported living for people with physical disabilities, learning disabilities and mental health issues.  With a regular caseload of around 60, you will also take on new cases from hospitals or community teams.  You’ll carry out holistic assessments and agree support plans that respect the independence, dignity and privacy of individuals and their carers.

 

With scope to manage your own caseload, you’ll also be part of a supportive team of eight Placement Monitoring Officers from various health and social care backgrounds. This allows us to share knowledge and experience to develop our expertise and provide the best possible service.

About You:

You’ll have a professional qualification in social work, occupational therapy or nursing and a good understanding of Adult Social Care, including legislation, statutory guidance and best practice. Experience of working across different agencies and teams to deliver services is essential.  You’ll also have a good understanding of how to work with metrics and other performance measures to shape and drive excellent services.
