The Role

 

When you join us in this important role you’ll work across the bi-borough of Kensington and Chelsea and Westminster, engaging with service users who have health and substance use co-morbidities. Your primary focus will be holding a caseload of service users, maintaining a safe, caring and therapeutic environment while helping them improve their lives, offering assessments, care planning, harm minimisation advice, structured sessions and group work. You will work in an assertive manner, rolling with ambivalence to offer support to service users who can be difficult to engage. 

 

Offering the highest standards of professional practice, it will be your task to assess, plan, implement and evaluate the service user’s needs, and where appropriate refer them to day or residential rehab programmes. Liaising with care co-ordinators and other professionals, you’ll also provide comprehensive, culturally appropriate, evidence-based assessments to establish their eligibility for health and social care services. 

 

Your work will involve the use of evidence-based social work, nursing and other psycho-social interventions including motivational interviewing, harm minimisation and relapse prevention – much of this will be offered to users in their own homes or other community settings. We’ll also expect you to offer training and guidance to external agencies, ensure the users’ cultural, spiritual and communication needs are addressed, and be ready to participate in a multi-disciplinary response to crisis situations.

About You

 

With a degree in Social Work or Nursing, or a qualification in Occupational Therapy or Clinical Psychology, you’ll be registered with a relevant governing body and possess some post-registration experience. Skilled at working in an inpatient or community setting with service users who have severe and enduring mental health problems or substance abuse issues, you’ll be capable of assessing, designing, delivering and evaluating appropriate health and social care packages.

 

Resilient, confident and ready to be an advocate for users who may not always want help, you’ll be patient and empathetic as the journey you’ll be taking might be a long and challenging one. With this in mind you’ll be a collaborative team-worker and capable of using a range of therapeutic skills and psycho-social interventions as you build warm and supportive relationships with users, carers, their families and a range of statutory and non-statutory agencies.
