The Role:

Joining our dedicated Community Response Team as a Reablement Team Leader, you’ll help to support adults across Hampshire who require additional care and support. Our Community Response Service focuses on short-term assessment and reablement for those with short or long term conditions and frailties as well as supporting those coming out of hospital. We are committed to promoting independence and enhancing the quality of life for our service users, as well as reduce, avoid or delay the need for ongoing care whilst supporting them to remain living at home.  

 

You’ll play a crucial role in the triage process, working in partnership with health colleagues to receive and assess referrals for the Reablement Team. Using a strengths-based approach, you’ll assess service user needs, undertake risk assessments, identify goals and coordinate care to promote daily living skills and independence. You’ll use your excellent communication, interpersonal and teamwork skills to establish effective working relationships with our external partners, including the Frailty Support Service, integrated intermediate care hubs and hospitals, to support discharge pathways and avoid hospital admissions. 

 

As a Reablement Team Leader you’ll be responsible for overseeing a team of Community Reablement Assistants, ensuring the delivery of high-quality reablement services. You’ll provide people management support to the team, motivating and coaching others to ensure we deliver the best service possible.

Working Patterns 
Our Service operates 7 days a week, 365 days of the year. You’ll be required to work a shift pattern that includes early, day and late shifts, in addition to weekends on a rotational 2:4 basis, including bank holidays, for which enhancements are paid. The current working hours for this position are subject to change. 

 

Other roles you may have experience in may include: Senior Case Worker, Rehabilitation Worker, Community Support Worker, Community Response Assistant, Senior Healthcare Assistant, Senior Care Assistant, Assistant Team Leader.  

Travel Requirements

You will be required to travel to various locations within Hampshire and it is essential that you have a current drivers’ licence and access to a reliable vehicle on a regular basis.
