The Role:

Joining our Supported Accommodation team as a Senior Case Worker supported by qualified social/health care professionals, you’ll assess the needs of adults in the community and provide them with support and guidance to maximise their wellbeing. Using a person-centred approach, you'll use your knowledge of strengths-based practice to focus on people’s resourcefulness and abilities and enable them to develop the best strategies to face life’s challenges and live independently and safely in the community.

In this role you’ll use your strong relationship management and negotiation skills to collaborate with individuals, their families, providers, and other health care professionals.

Travel Requirements:
