About The Role:

As a Family Practitioner, you will play a crucial role in supporting children with disabilities and their families. You will:

  • Manage specific cases, ensuring tailored support and interventions
  • Collaborate with social workers and other professionals to assess needs and develop comprehensive family plans
  • Deliver a range of tasks and interventions within our strengths-based Hampshire Approach framework

 

What We Are Looking For:

  • We are seeking practitioners with experience working with children with disabilities and their families
  • Strong skills in motivating and engaging families to achieve positive outcomes
  • Good communication and IT skills
