Salary range: £21,403 - £24,055 per annum

The Role:

This is an exciting opportunity to join our Physical Disability service for younger adults, working in our PD Central Team, our nominal office base is in Romsey. 

The service is made up of three teams working with adults with physical disabilities and long term conditions as part of the wider Younger Adults service.

In this role you will work collaboratively with partner agencies to promote the independence and improve the wellbeing  of younger adults with physical disabilities  living in the community. Working in accordance with social care legislation and local government requirements, informed by the values and principles of social care, you will be supporting in the delivery of person centred, safe and effective social care services, demonstrating excellent strength based practice.

Our priority is to safeguard the most vulnerable and to be as efficient as possible, whilst utilising the opportunities afforded by closer working with partner agencies, working proactively to promote seamless transitions in care and safe living within the community. You will have excellent case management skills or relevant transferable skills, as well as risk assessment skills.  The ability to work in partnership with individuals to identify their care and support needs is vital to the role and will draw upon your excellent communication skills.

You will receive regular supervision by experienced practitioners who have excellent operational understanding and who understand our unique local challenges. Working across a wide geographical area, the teams will work in a mobile and flexible way, making best use of home-working, HCC hubs and drop-ins. As such, you will need to be confident in working autonomously and willing to embrace full use of digital technology.
