Be a Social Worker for Hampshire County Council and practice in an Outstanding Authority

We are seeking experienced and passionate Social Workers to join our innovative Out of Hours team,based at Hampshire County Council’s frontline contact centre in Fareham. Our Out of Hours team provides triage and multi-agency assessment of safeguarding concerns. Working closely with the Contact Assessment Resolution Team, we work to protect the most vulnerable Adults from harm, neglect and abuse.

Using your professional knowledge and experience you’ll make assessments and decisions about the most appropriate interventions. Applying your significant Social Work experience and excellent communication skills you’ll establish and maintain relationships to ensure services meet identified needs.

You’ll be committed to facilitating effective multi-agency solutions to ensure positive and sustainable outcomes. Using best practice, you’ll strive to provide the best outcomes for vulnerable adults both young and elderly. Most importantly however, you’ll have a passion for supporting people in the community and in residential care.

Flexibility in working hours is key to ensure services are delivered at optimal times of the day to reflect the needs of vulnerable people. Therefore, you will be required to work outside of standard business hours including Bank Holidays, evenings, and weekends. As you will be working with families across a geographical area, you must be able to meet the travel requirements of the role.

Role Responsibilities:
  • Use skills, knowledge, and experience to make full assessment of risks protection and social care needs.
  • Manage an agreed case load.
  • Use specialist communication skills to achieve optimum outcomes for service users.
  • Devise individual care plans, taking account of diversity and promoting dignity and empowerment.
  • Offer advice and support to unqualified staff and students to assist development.
  • Work with other agencies and stakeholders to develop partnerships and to undertake joint assessments where appropriate
About you

You’ll be:

  • A qualified Social Worker registered with HCPC.
  • Experienced in working with Vulnerable Adults both young and elderly.
  • Experienced in working in a multi-disciplinary, interagency basis.
  • Creative, flexible and forward thinking in your approach.
  • Knowledgeable of up to date legislation and understanding of safeguarding issues.

  • Constantly striving for continuous improvement to provide the best outcomes for people in need.
  • Able to use your excellent mediation and negotiation skill on behalf of service users.
