About Us:

THE EXTRAORDINARY STORY OF INDEPENDENT LIVES

Adult Social Care in Westminster City Council is a world of extraordinary stories, where our passionate, caring and committed professionals do brilliant work every day.

Mrs D was frail and unwell. Her son was her main carer, but he wasn’t coping. We stepped in to help, but they were hostile to outside agencies. They’d always lived together and were terrified they’d be split up. Our social worker did a great job persuading Mrs D to get hospital treatment and a temporary stay in a care home. In an epic effort throughout COVID, she gained the son’s trust and sorted all the repairs, carers and nurses needed so that Mrs D could come home to live with her son. We also got Mr D regular respite and a personal budget awarded in recognition of him as her carer. After all, there’s nothing more important than living the life you want.

The Role:

As a Social Worker in our CIS (Community Independence Service) Reablement team, you can make your own powerful contribution by helping adults regain their independence and maintain it for as long as possible.

Working with a manageable caseload of older people and adults with physical disabilities, you will assess their needs both at home and in community settings. Identifying the measures they need to live safely and well, you will create comprehensive support plans that are realistic, and you will help service users choose the most relevant options that are suited to their needs.

While a vulnerable adult’s immediate situation is obviously of vital importance, you will also look to the future to identify any integrated health and social care solutions that could be implemented now, in order to delay future needs from occurring. With a focus on safeguarding everyone that you’re working with (both service users and anyone they’re living with), you will devise achievable protection plans.

This is a valuable opportunity to make a tangible impact on the emotional and physical wellbeing of Westminster’s residents. With the full support of a multidisciplinary team that includes Occupational Therapists and Independent Living Assessors, you will provide a valuable backbone of support to service users’ families and carers, listening to their concerns and signposting them to the relevant support and information that will provide a better quality of life for both them and the people they’re caring for.

Please refer to the Job Description for more information.

About You:

This is a great chance to pursue your ambitions in a team who want you to succeed and progress in your career. To be equal to the challenge, you will therefore be a qualified and registered Social Worker, with experience of supporting adults within the community, and ideally within their own home environments. Highly organised with a current enhanced DBS check in place, you’ll have a good awareness of the relevant legislation and safeguarding procedures.

From a practical point of view, you’ll have experience of completing Mental Capacity Assessments and creating support plans, so you’ll also have a good awareness of simple equipment options and support services that will enable people to remain safely within their preferred living environment.

Able to accurately record your assessments and recommendations, you’ll be competent in using a range of IT programmes and software including Mosaic, System1 and MS Office.

At the heart of this role is the need to work with people from a wide variety of cultural and ethnic backgrounds, so you’ll have exceptional problem-solving and communication skills.
