Reablement Practitioner/Therapy Assistant

We are seeking skilled, enthusiastic, and self-motivated Reablement Practitioners to join our Adults’ Health and Care Reablement Service in Hampshire on a temporary basis until end of March 2022. We have multiple vacancies across the county (within South, West, North and Mid Reablement Hubs) on a full and part time basis.

The Reablement Practitioner/Therapy Assistant roles will be specifically supporting the management and flow of a Rapid Discharge Service, providing a short-term reablement and domiciliary service, offering timely hospital discharge for individuals who may have an eligible Care Act need. These individuals may need support with discharge from a hospital setting to their own home or may require support to prevent a hospital admission, thereby supporting flow and effective management within the wider system.

You will be multi skilled in understanding and completing the care management process and delivering therapy-based interventions with support from professionally qualified staff.

You will be part of a progressive team, using an enabling, adaptive approach, supporting people to live in their own homes where possible, and in the least restrictive way.

You will be able to demonstrate effective assessment and treatment planning skills, goal setting and, work in a fast paced, evolving environment. You must be able to demonstrate strengths-based approach to provide advice and information to enable service users to problem solve their own needs and make informed decisions on their Reablement journey.

You will have excellent communication, inter-personal and team player skills.

Travel within Hampshire may be required, you will need to respond flexibly to these requirements. This post may potentially require working over a 7-day pattern, so flexibility is needed.

There may also be opportunities to work across Hampshire as part of a county wide Reablement Service comprising Occupational Therapy, Sensory Services and CQC Registered Community Response Staff.

Previous experience in a Reablement, Rehabilitation or Social Care setting is required.

The role may include the supervision and management of staff.

North Hub - Basingstoke covering Alton, Aldershot, Liphook, Whitchurch, Bordon, Farnborough
Mid Hants Hub - Winchester covering Bishops Waltham, Andover, Romsey, Hedge End, West End
South Hub - Fareham covering Gosport, Havant, Petersfield, Horndean, Emsworth, Hayling Island, Porchester, Salisbury Green.
West Hub - Totton covering New Forest, North Baddesley, Eastleigh, Chandlers Ford
