Community Reablement Opportunity

Hampshire County Council's Reablement Team have a fantastic and exciting opportunity to join us as Community Reablement Assistant - Level 2.. The team provides Reablement in the community to frail and vulnerable Adults across Hampshire. The team includes the Community Response Team, Occupational Therapists and Sensory Officers. We are passionate about our Staff’s professional development and there are great opportunities for various avenues of career progression across all our disciplines, which include the option of becoming a qualified practitioner. 

The Reablement team will be working in partnership with our Health colleagues to help reduce the need for the service user to be admitted to hospital. You will provide Reablement by offering encouragement and guidance to maximize the independence and potential, by helping service users to remain living in their own home. This will include intimate personal care, continence care, Moving and Handling to continuously work towards the service user’s independence.  

What you will be doing:
Your responsibilities will be to ensure that initial visits with service user’s which include risk assessments, goal setting and updating the service user’s reablement plans are completed for all our CRA1’s to follow. There is an element of personal care to be provided within this role, you will also coach and mentor CRA 1’s to ensure staff meet levels of expected competence. CRA 2’s also support Team Leaders, Occupational Therapists and Sensory Officers within their daily responsibilities.  

You will be expected to work on a rolling roster, which includes AM’s, PM’s and weekend shifts. Service hours are between 07:00-22:00, 365 days of the year.  

You will need to travel across the area and you will be paid travel from your office base.  

The Reablement Service:
The Reablement Team provide a full an in-depth induction programme with continuous professional training and development. Together with full support from Managers, Team Leaders, Occupational Therapists and Sensory Loss Officers who will provide coaching and mentoring.There are opportunities for quick progression and fantastic prospects for career development. Some of our past Community Reablement Assistants have even gone on to becoming Occupational Therapists or Specialist Sensory workers.

We will also help you to gain a National Professional qualification and develop your career within Adults’ Health and Care.

Our Community Reablement Assistants underpin our service and we are continually seeking opportunities to enhance delivery. We are passionate about ensuring personal development for all staff and have a structured workforce development plan whereby we actively encourage and support continuing professional development, including formal qualification routes.
