We have an exciting opportunity available within the Hampshire County Council Adults' Health & Care Reablement Service. We are seeking highly skilled, enthusiastic and self-motivated Occupational Therapists to join our service in the North of Hampshire.

As a qualified Occupational Therapist, you will be responsible for leading holistic assessments of Reablement needs and planning Reablement interventions to support across a multi-disciplinary team.

There may also be opportunities to work across Hampshire as part of a county wide Reablement Service comprising Occupational Therapy, Sensory Services and CQC Registered Community Response Staff. We are currently working to develop strong links with our colleagues in Health, such as integrated intermediate care hubs and hospitals, to support discharge pathways and avoiding unnecessary hospital admissions. You will be instrumental in the success of this service development.

You will be able to demonstrate effective assessment and treatment planning skills, goal setting and, work in a fast pace community service.You must be able to demonstrate strengths-based approach to provide advice and information for clients to problem solve their own needs and make informed decisions on their Reablement journey.

You will have excellent communication, inter-personal and team player skills.There is a responsibility to supervise junior Reablement staff within the team.  Previous experience in a Reablement or Rehabilitation setting would be advantageous but not essential.

Job Purpose:

To work collaboratively with a multi-disciplinary team in order to identify and respond to the needs of people with disabilities, enabling them to live as independently as possible within their own homes.

To coordinate a specific programme of intervention in the community if needed and primarily working in our bed based reablement service.

Role Responsibilities:

Assessment

  • Assessing users to participate and maintain maximum personal independence
  • Assessing carers’ needs and abilities to support people with disabilities

Recording

  • Written recording in line with departmental guidelines
  • Maintaining case records and reports
  • Inputting and amending computer records

Advising

  • Providing information to duty and other professionals / teams
  • Advising people on rights and benefits
  • Sign-posting people to correct location to obtain service required
  • Advising on appropriate choice of special equipment, adaptation and housing needs

Communication

  • Verbal and written communication with other professionals and the public
  • Representative at multi-disciplinary meetings
  • Email and system searches

Planning

  • Ability to organise work in relation to priority of workload
  • Particularly relevant to DFG applications and reablement intervention

Technical

  • Read and adapt plans for adaptations and advise on the needs of disabled people with a view to adapting their home to maintain independence

Training

  • Provide on going training and support to Occupational Therapy Assistants
  • Training in manual handling
  • Training in use of equipment
  • To maintain and update level of knowledge

Corporate and statutory initiatives – equalities / health and safety / e-government / sustainability

  • Maintain an awareness of corporate and departmental policies and procedures and ensure these are adhered to in the workplace
  • Ensuring appropriate training for self and staff
Role Requirements:
  • You must be OT qualified
  • You must be registered with HCPC
  • Previous experience of working in the community or hospital base
  • Ability to practical problem solving
  • Continuing professional development (CPD) to keep your skills and knowledge up to date and are able to work safely, legally and effectively.
  • Work effectively as a multi-disciplinary team member
  • Understand the principal of independence
  • Experience of moving and handling of people
  • Knowledge of aids, equipment, housing
  • Time Management – Ability to manage demanding caseload
  • You must be able to meet the travel requirements of the role
  • Proficient IT Skills
