Our service aims to enable adults with a learning disability to engage in meaningful activities and to establish a valued place within their community.

You will be supporting people to engage in activities of their choice, with a key principle of accessing activities within the local community; supporting people to develop community networks and you'll get great satisfaction from supporting people to engage in our sensory activities in-house. You will also be providing person centred support to people who need help with things we take for granted such as personal care & communication. 

You'll likely have some experience of enabling and providing care, but most importantly you'll share our commitment to supporting people. You'll be a strong team player, with great communication skills, including some IT and recording skills. HCC have a comprehensive induction programme that will provide you with all the training you need.

Job Purpose:

To provide people with more complex needs to have a stimulating day that meets their needs and to provide reactive, flexible support throughout the day as peoples needs change.

Role Responsibilities:

To assist in providing support to service users

  • Assist service users with eating, drinking, personal care and any other identified support need, as necessary / as per All About Me Book while maintaining the persons dignity, respect and safety to keep people comfortable throughout the day
  • To follow agreed methods of meeting the needs of service users, deviating from these only after consultation with the line manager
  • Recording day to day information clearly and concisely on the service users experience in the service including incident reporting, behavioural monitoring or daily communication books
  • To communicate any changes relating to service users, that may affect their service, to line manager in both verbal and written form as appropriate
  • To assist the Day Opportunities Team Co-ordinator in the assessment and reviewing of the strengths, needs and aspirations of services users and to give feedback when appropriate either in writing or by attending reviews
  • To work with other colleagues and the Day Opportunities Team Co-ordinator to determine the personal outcomes for service users and assist in the development of a care plan to meet those assessed needs i.e. All About Me Book
  • Communicate positively with all service users and develop alternative forms of communication where necessary following the individual communication passports
  • To support people with mobility or other physical difficulties arising from physical or sensory impairments in a way that maximises independence
  • Enable service users to recognise poor support and take appropriate action and to act on their behalf if there are safeguarding issues by taking the issue to your line manager if appropriate
  • Support service users to prepare for and participate in any meetings they may attend
  • Support service users to manage their medication as appropriate and complete records


To assist in the planning, development and access to activities

  • Support service users to make use of community resources to enable community participation and to promote social inclusion through exercising choice
  • Work with service users to develop their knowledge of local resources and facilities on offer to enable them to make an informed choice and facilitate agreed experiences
  • Be an active part of the planning of the programme of activities for service users each day/week with the rest of the staff team
  • To escort or transport individuals to and from services as required, driving may be required if appropriate once MIDAS trained
  • To record the evaluation of activities/sessions for groups/service under the direction of the Day Opportunities Team Co-ordinator

Additional Responsibilities

To work co-operatively with others

  • Under the direction of the line manager, to work and liaise with parents / carers and staff from other agencies and organisations
  • To act as Link worker with families/carers and pass on factual information as necessary regarding individual service users under the direction of the line manager with the understanding that more detailed or complicated information is passed on by more senior staff
  • To act as a professional and effective member of the staff team and promote a positive image of Hampshire County Council Day Services


To further own personal and professional development

  • To actively participate in team/staff/planning meetings as required
  • To participate in supervision and performance development reviews which will be undertaken by the Line Manager
  • To undertake all mandatory training (i.e. SCIP, medication management, moving and handling, etc.) and training identified with line manager which is appropriate to the role
  • To have the opportunity to undertake Level 3 training eg Health and Social Care through identification through Valuing Performance


To ensure the health, safety and wellbeing of service users within the unit and in the community

  • To assist the Day Opportunities Team Co-ordinator in completing and updating full and comprehensive risk assessments
  • To adhere to risk assessments and safe methods of work and to understand and comply with all procedures relevant to the role
  • To work in accordance with Hampshire County Council, Adults’ Health & Care Services 
  • Department and unit policies and procedures at all times, seeking advice as necessary
  • To share responsibility for the general welfare of everyone who uses the service
  • To be responsible for the safety, tidiness, cleanliness and security of work areas and for the care of all equipment in those areas
  • Take reasonable care of own health and safety and that of others who may be effected by acts or omissions
  • Use specific equipment and/or clothing supplied through Health & Safety assessment procedures for own and other’s protection

Role Requirements:

  • Good standard of education
  • Ability to work with service users in line with current service philosophy
  • Good general communication skills
  • Ability to undertake Level 3 training eg Health and Social Care
  • Ability to communicate with service users with communication difficulties and know when it is appropriate to advocate on their behalf
  • Basic written and numerical skills, some experience of record keeping
  • Some experience or understanding of working in a day service environment and/or working with service users with high support needs and/or challenging behaviour
  • Some experience or knowledge of risk management
  • To be able to work as part of a team and form relationships with service users
  • A flexible and positive approach to meet the changing needs of the service and people who use it
  • Ability to work without direct supervision and to organise and prioritise own work load
  • Commitment to and understanding of equal opportunities issues
  • Commitment to attend staff training and participate in own development through supervision and performance development
  • Attendance of HCC Corporate and Departmental induction
  • To take reasonable care to promote a healthy working environment and safe working practices in accordance with HCC Health and Safety Policy

  •  Driving (and MIDAS training) may be required in some roles
