The Role:

 

From the moment you join our Assessment and Care Management service, you will have the opportunity to thrive. A robust induction is followed by ongoing training and development, as well as regular supervision. In this front-facing role, you will be required to visit our residents in the community to assess their needs and complete various assessments including safeguarding. You will occasionally respond to urgent situations. One of the key challenges will be managing and prioritising your own caseload of assessment work. It’s a busy team but you’ll be surrounded by supportive colleagues and managers who are dedicated to helping you do your best work.

About You:

 

A deep understanding of Social Work and the credibility this gives are key here. This will underpin our work. Your flexible approach will reflect our own. Yours too will be empowering by being a constant presence to support the team and bringing critical insight into what needs to happen to deliver the outcomes for which we all strive. Your proven experience means you’ll understand the relevant legislation inside out, not to mention risk management, safeguarding and care planning. Crucially, you will have excellent analytical report writing skills, with the communication skills needed to build trust with residents.
